Sports Physical Day

The sports physical day is an oppertunity for all athletes to complete their SportWare and physical form for the 2017-2018 athletic season. Cost: $20.00 (Cash or Check only)
What to bring: SportsWare Health History Form (Completed & Printed) and insurance card
What to wear: Athletic clothing and footwear
No addmittance after 8:30 pm
If you wish to volunteer, we are in need of physicians, nurse practitioners or physician assistants. Please contact Sarah Merkel at smerkel@justin-siena.org.
